First Visit and Care
What should I bring to my first appointment?
For your first visit, please bring your ID, insurance card, a list of your current medications, and any past medical or immunization records. Please arrive 15 minutes early to finish your paperwork.How do I get a prescription refill?
The fastest way to get a refill is to call your pharmacy directly. They will send a request to our office. Please allow 24 to 48 hours for our team to check and approve your refill.
